Form 1040-V

Form 1040-V, Payment Voucher

By RepeatForm team

Form 1040-V is a statement you send with your check or money order for any balance due on the "Amount you owe" line of your Form 1040, 1040-SR or 1040-NR, says the IRS. RepeatForm reuses your name, address and social security number: upload last year's voucher and a blank, then review.

What Form 1040-V is for

The IRS form says "What Is Form 1040-V? It's a statement you send with your check or money order for any balance due on the 'Amount you owe' line of your 2025 Form 1040, 1040-SR, or 1040-NR." It also says "If paying online at www.irs.gov/Payments, don't complete this form." RepeatForm does not decide whether you owe tax or how to pay it.

The four lines

The form's own instructions: "Line 1. Enter your social security number (SSN). If you are filing a joint return, enter the SSN shown first on your return." "Line 2. If you are filing a joint return, enter the SSN shown second on your return." "Line 3. Enter the amount you are paying by check or money order." "Line 4. Enter your name(s) and address exactly as shown on your return. Please print clearly."

Preparing your payment

The form says to make your check or money order payable to "United States Treasury" and "Don't send cash." It adds: "Don't staple or otherwise attach your payment or Form 1040-V to your return." The voucher itself says "Do not staple or attach this voucher to your payment or return." For where to mail the return, payment and voucher, use the mailing list on the form.

Using the same details next year

Upload the Form 1040-V you completed last year and this year's blank; RepeatForm carries the matching answers across; you review them, then preview the PDF before you export. Your name, address and social security number are the part you save retyping.

What carries over

  • ✓Your social security number, where it matches in both copies
  • ✓Your spouse's social security number (line 2), where it matches in both copies
  • ✓Your name(s) and address (line 4), where they match in both copies
  • ✓Anything else that matches in both copies. Check it yourself; the amount you are paying changes every year

What never carries over

  • ✓The amount you are paying (line 3). It describes this year's balance, so enter it yourself.
  • ✓Anything the IRS prints and locks on the form, and anything in your completed copy that does not appear in the blank form.
  • ✓A signature or date. The PDF has no field for either.
  • ✓Making the payment. A completed voucher is not a payment: pay as the form describes.

If the IRS has a new edition

The IRS publishes a Form 1040-V for each tax year. Carrying a prior year's copy into the new one works for many answers, but some fields are renamed or moved and match only partly, so check every line on the new year's form in Review before you export.

Estimated payments for next year are on Form 1040-ES.

The payment is for the balance on Form 1040.

Good to know

  • ✓You review every carried-over answer. RepeatForm does not guarantee the form is correct and cannot promise the IRS will accept it.
  • ✓RepeatForm does not give tax or legal advice. It cannot tell you whether you owe tax, how much to pay, or whether to use a voucher. Use the IRS instructions or a qualified adviser.
  • ✓RepeatForm does not make or send a payment or file anything for you.
  • ✓It works with digital, text-selectable PDFs only. Scans, photos and handwriting are not supported.
  • ✓Same or recent editions work best. If the form changed you may get fewer matches, so check the preview carefully and get the current form from irs.gov.

FAQ

FAQ

What is Form 1040-V?+

The IRS form says "It's a statement you send with your check or money order for any balance due on the 'Amount you owe' line" of your Form 1040, 1040-SR or 1040-NR. RepeatForm helps you fill in the voucher; it does not make the payment.

Do I need to file Form 1040-V?+

The form says "If paying online at www.irs.gov/Payments, don't complete this form." It is for payment by check or money order. Whether you need to pay by check is for you to decide; RepeatForm cannot advise.

Should I staple Form 1040-V to my check?+

The IRS says no: "Don't staple or otherwise attach your payment or Form 1040-V to your return," and the voucher says "Do not staple or attach this voucher to your payment or return."

Can I reuse a Form 1040-V I filled out before?+

You can reuse your name(s), address and social security number(s) where they match. The amount you are paying changes each year, so enter it again and check every line in Review.
