Form W-4R

Form W-4R, Withholding Certificate for Nonperiodic Payments and Eligible Rollover Distributions

By RepeatForm team

Form W-4R tells the payer of a nonperiodic payment or eligible rollover distribution how much federal income tax to withhold. The form says to give it to the payer of your retirement payments. RepeatForm helps when you give one to more than one payer: upload a W-4R you completed and a new blank, then review.

Tested with the irs.gov edition dated 2026

Is this page for you?

Plainly: this is a lower-fit page. A W-4R usually goes to a payer once, and the form says your choice "will generally apply to any future payment from the same plan or IRA." Reuse helps most if you give a W-4R to more than one payer or for more than one distribution, or help someone with their own copy.

What Form W-4R is for

irs.gov says "Complete Form W-4R so your payer can withhold the correct amount of federal income tax from your nonperiodic payment(s) or eligible rollover distribution from an employer retirement plan, annuity (including a commercial annuity), or individual retirement arrangement (IRA)." The form itself says "Give Form W-4R to the payer of your retirement payments." RepeatForm does not work out any amount or rate on it.

Form W-4R, Form W-4P and Form W-4

The form says "Don't use Form W-4R for periodic payments," and for those points to "Form W-4P, Withholding Certificate for Periodic Pension or Annuity Payments." Form W-4 is the certificate you give an employer for a job. Each has its own RepeatForm page, linked below.

Default rates and line 2

The form says "For nonperiodic payments, the default withholding rate is 10%" and "For an eligible rollover distribution, the default withholding rate is 20%." Line 2 is where you enter a different rate, and the form prints marginal rate tables to help. For eligible rollover distributions it says "Don't give Form W-4R to your payer unless you want more than 20% withheld." The choice is yours: RepeatForm does not suggest a rate.

If you do not give one to your payer

The form says "If you don't give Form W-4R to your payer, you don't provide an SSN, or the IRS notifies the payer that you gave an incorrect SSN, then the payer must withhold 10% of the payment for federal income tax and can't honor requests to have a lower (or no) amount withheld." It also says "Submit a new Form W-4R if you want to change your election." The form is signed: "Your signature (This form is not valid unless you sign it.)"

Using the same details for another payer

Upload a Form W-4R you completed and the new blank; RepeatForm carries the matching answers across; you review them, then preview the PDF before you export. Use one prior copy as the source for each new payer's form, then change what differs.

What carries over

  • ✓Your name and address (line 1a), where they match in both copies
  • ✓Your social security number (line 1b), where it matches in both copies
  • ✓Anything else that matches in both copies. Check it yourself; your payment and your choices can differ for each payer

What never carries over

  • ✓The rate on line 2. It is your decision for this payment and this payer, so set it yourself.
  • ✓Anything the IRS prints and locks on the form, and anything in your completed copy that does not appear in the blank form.
  • ✓A signature or date. The PDF has no field for either, so sign and date by hand: the form is not valid unless you sign it.
  • ✓Giving the form to your payer. A completed form does nothing until your payer has it.

If the IRS has a new edition

The IRS publishes a new Form W-4R when the form changes. Carrying an earlier copy into the new one works for many answers, but some fields are renamed or moved and match only partly, so check every line on the new form in Review before you export.

For periodic pension or annuity payments, see Form W-4P.

The withholding certificate for a job has its own page: Form W-4.

Good to know

  • ✓You review every carried-over answer. RepeatForm does not guarantee the form is correct and cannot promise your payer will accept it.
  • ✓RepeatForm does not give tax or legal advice. It cannot tell you what rate to choose or whether a payment is a nonperiodic payment or an eligible rollover distribution, and it does not work out any amount. Use the IRS instructions or a qualified adviser.
  • ✓RepeatForm does not give the form to your payer or file anything for you.
  • ✓It works with digital, text-selectable PDFs only. Scans, photos and handwriting are not supported.
  • ✓Same or recent editions work best. If the form changed you may get fewer matches, so check the preview carefully and get the current form from irs.gov.

FAQ

FAQ

What is a W-4R form?+

irs.gov says "Complete Form W-4R so your payer can withhold the correct amount of federal income tax from your nonperiodic payment(s) or eligible rollover distribution from an employer retirement plan, annuity (including a commercial annuity), or individual retirement arrangement (IRA)." The form says to give it to the payer, not to the IRS.

What is the difference between a W-4, W-4P and W-4R?+

Form W-4 is the withholding certificate you give an employer for a job. Form W-4P is for periodic pension or annuity payments, and Form W-4R is for nonperiodic payments and eligible rollover distributions: the W-4R says "Don't use Form W-4R for periodic payments" and points to Form W-4P instead. RepeatForm has a page for each of the W-4 and W-4P.

How do I fill out Form W-4R?+

Line 1a is your name and address and line 1b your social security number. Line 2 is only for a rate different from the default: the form says to "Enter the rate as a whole number (no decimals)." You then sign. The form prints marginal rate tables and a page of instructions to help you choose; RepeatForm does not choose a rate for you.

Can I reuse a Form W-4R I filled out before?+

You can reuse your personal details, such as your name, address and social security number, when you give a form to another payer. The rate and your choices can differ, so enter them again and check every line in Review. Sign and date it by hand, then give it to your payer.
