Form 4506-C IVES Request for Transcript of Tax Return

By RepeatForm team

Form 4506-C is the IRS form used to request tax return information through an authorized IVES participant, whom you name on line 5a. The IRS says not to sign it until every applicable line is complete. If you repeat the same participant and client lines on more requests, upload a 4506-C you completed and a blank one. You review, collect the signature yourself, and preview free.

What Form 4506-C is

The form's instructions say to use Form 4506-C to request tax return information through an authorized IVES participant, and that you designate that participant to receive the information on line 5a. The form also carries a caution: the tax transcript is sent to the third party entered on line 5a and/or 5d, so make sure lines 5 through 8 are complete before signing. RepeatForm cannot tell you whether you should sign one or who should receive your information.

Who signs, and the 120-day rule

The form says that if the request applies to a joint return, at least one spouse must sign, but if both spouses' names and taxpayer identification numbers are listed, both must sign. Someone other than the taxpayer, such as a corporate officer, partner, guardian or executor, certifies they have the authority to sign. The form says it must be received by the IRS within 120 days of the signature date. There are also two boxes: one for a form signed by an authorized representative, and one confirming the document was signed electronically. The IRS's IVES FAQ says a form signed by an authorized representative without that box marked will be rejected, and the same for an e-signature without its box. Tick those only if they are true.

Which lines are which

Lines 1 to 4 are the taxpayer's name, taxpayer identification number (SSN, ITIN or EIN), previous name, and current and previous addresses. Line 5 holds the IVES participant's details (5a), an optional customer file number (5b) and unique identifier (5c), and the client company's name, phone and address (5d). The instructions say line 5d cannot be blank or "NA". Line 6 is for the type of transcript for one tax form number per request: a return transcript, an account transcript or a record of account. Line 7 is for wage and income transcripts, and line 8 for the year or period. RepeatForm does not choose any answer: read the instructions for each line.

Using the same details on a new copy

Upload a 4506-C you completed and a blank one; RepeatForm carries the matching answers across; you review them, then preview the PDF free.

What carries over

  • ✓Name, previous name, address and previous address, where they match in both copies
  • ✓The IVES participant block (line 5a) and the client block (line 5d), where they match in both copies, which is the part most repeated by lenders and processors. Check each one
  • ✓Anything else that matches in both copies. Check the transcript type, form number and year yourself, because they change with each request

What never carries over

  • ✓The taxpayer's signature and the spouse's signature. The signature boxes stay blank for the right people to sign. The dates and the signing-method boxes can carry from your completed copy, so check each one and change it if it no longer applies.
  • ✓Anything the IRS prints and locks on the form.
  • ✓Anything in your completed copy that does not appear in the blank form.
  • ✓The 120-day clock. A new copy needs a new signature date.

If the IRS has a new revision

The IRS updates forms from time to time. Carrying an older copy into the current revision works for many answers, but some fields are renamed or moved and match only partly, so check every field after a new revision.

Good to know

  • ✓You review every carried-over answer. RepeatForm does not guarantee the form is correct and cannot promise the IRS will accept it.
  • ✓RepeatForm does not give tax or legal advice. It cannot tell you whether to sign, which transcript to request or who may receive your information. Use the IRS instructions or the party asking for the transcript.
  • ✓RepeatForm does not send the form to the IRS or to an IVES participant.
  • ✓It works with digital, text-selectable PDFs only. Scans, photos and handwriting are not supported.
  • ✓Same or recent revisions work best. If the form changed you may get fewer matches, so check the preview carefully and get the current form from irs.gov.
