Form W-4P

Form W-4P, Withholding Certificate for Periodic Pension or Annuity Payments

By RepeatForm team

Form W-4P tells the payer of your pension or annuity payments how much federal income tax to withhold. The form says to give it to the payer. RepeatForm helps when you update it or give one to more than one payer: upload a W-4P you completed and a new blank, then review.

Is this page for you?

Plainly: a W-4P is often given to a payer once and not touched again, so for many people the benefit is small. This page helps most if you are updating a W-4P, giving one to more than one payer (the form says "Submit a separate Form W-4P for each pension, annuity, or other periodic payments you receive"), or helping someone with their own copy.

What Form W-4P is for

irs.gov says "Complete Form W-4P so your payer can withhold the correct amount of federal income tax from your periodic pension, annuity (including commercial annuities), profit-sharing and stock bonus plan, or individual retirement arrangement (IRA) payments." The form itself says "Give Form W-4P to the payer of your pension or annuity payments." RepeatForm does not work out any amount on it.

Form W-4P, Form W-4 and Form W-4R

The form says not to use W-4P for a nonperiodic payment or an eligible rollover distribution (including a lump-sum pension payment): "Instead, use Form W-4R, Withholding Certificate for Nonperiodic Payments and Eligible Rollover Distributions". For the Form W-4 that goes with a job, the W-4P says: "If you (or if married filing jointly, you and/or your spouse) have a job(s), do NOT complete Steps 3 through 4(b) on Form W-4P. Instead, complete Steps 3 through 4(b) on the Form W-4 for the job." The Form W-4 has its own page, linked below.

If you do not give one to your payer

The form says "If you don't give Form W-4P to your payer, you don't provide an SSN, or the IRS notifies the payer that you gave an incorrect SSN, then the payer will withhold tax from your payments as if your filing status is single with no adjustments in Steps 2 through 4. For payments that began before 2026, your current withholding election (or your default rate) remains in effect unless you submit a new Form W-4P." The form also says "If your tax situation changes ... you should submit a new Form W-4P."

Completing the steps

The form says "Complete Steps 2–4 ONLY if they apply to you; otherwise, skip to Step 5." and suggests the estimator at www.irs.gov/W4App for the most accurate withholding. Step 5 is the signature: "Your signature (This form is not valid unless you sign it.)". RepeatForm does not fill amounts for you, does not choose which steps apply, and does not provide a withholding calculator.

Using the same details on a new copy

Upload a Form W-4P you completed and the new blank; RepeatForm carries the matching answers across; you review them, then preview the PDF before you export. Use one prior copy as the source for each new payer's form, then change what differs.

What carries over

  • ✓Your name and address (Step 1), where they match in both copies
  • ✓Your social security number, where it matches in both copies
  • ✓Anything else that matches in both copies. Check it yourself; your filing status, income and adjustments can change

What never carries over

  • ✓The year and everything that depends on it: the amounts in Steps 2, 3 and 4, such as income from a job or other pensions, credits, other income, deductions and extra withholding. Enter those again from your current records and the IRS instructions.
  • ✓The "No withholding" choice. It is your decision for this payer and this time, so set it yourself.
  • ✓Your filing status. If it changed, set it yourself.
  • ✓Anything the IRS prints and locks on the form, and anything in your completed copy that does not appear in the blank form.
  • ✓A signature or date. The PDF has no field for either, so sign and date by hand: the form is not valid unless you sign it.
  • ✓Giving the form to your payer. A completed form does nothing until your payer has it.

If the IRS has a new edition

The IRS publishes a new Form W-4P when the form changes. Carrying an earlier copy into the new one works for many answers, but some fields are renamed or moved and match only partly, so check every line on the new form in Review before you export.

The withholding certificate for a job has its own page: Form W-4.

Good to know

  • ✓You review every carried-over answer. RepeatForm does not guarantee the form is correct and cannot promise your payer will accept it.
  • ✓RepeatForm does not give tax or legal advice. It cannot tell you how much to have withheld or which steps apply to you, and it does not work out any amount. Use the IRS instructions or a qualified adviser.
  • ✓RepeatForm does not give the form to your payer or file anything for you.
  • ✓It works with digital, text-selectable PDFs only. Scans, photos and handwriting are not supported.
  • ✓Same or recent editions work best. If the form changed you may get fewer matches, so check the preview carefully and get the current form from irs.gov.

FAQ

FAQ

Do I have to file a W-4P?+

The form says to give it to your payer, not file it. It also says "If you don't give Form W-4P to your payer, you don't provide an SSN, or the IRS notifies the payer that you gave an incorrect SSN, then the payer will withhold tax from your payments as if your filing status is single with no adjustments in Steps 2 through 4." For payments that began before 2026, the form says your current election or default rate stays in effect unless you submit a new one.

What is Form W-4P used for?+

irs.gov says "Complete Form W-4P so your payer can withhold the correct amount of federal income tax from your periodic pension, annuity (including commercial annuities), profit-sharing and stock bonus plan, or individual retirement arrangement (IRA) payments." For a nonperiodic payment or an eligible rollover distribution, the form says to use Form W-4R instead.

How do I complete the W-4P form?+

The form says "Complete Steps 2–4 ONLY if they apply to you; otherwise, skip to Step 5." Step 1 is personal information, and Step 5 is your signature, which makes the form valid. It suggests the estimator at www.irs.gov/W4App for the most accurate withholding. RepeatForm does not work out amounts or choose steps for you.

What is the difference between a W-4 and a W-4P?+

Form W-4 is the withholding certificate you give an employer for a job, and Form W-4P is the certificate for periodic pension or annuity payments. If you have a job, the W-4P says to complete Steps 3 through 4(b) on the Form W-4 for the job instead of on the W-4P. RepeatForm has a page for the W-4.

When should I submit a new Form W-4P?+

The form says "If your tax situation changes, or you chose not to have federal income tax withheld and you now want withholding, you should submit a new Form W-4P", and to "Submit a separate Form W-4P for each pension, annuity, or other periodic payments you receive." Read the form's instructions for your case.

Can I reuse a Form W-4P I filled out before?+

You can reuse your personal details, such as your name, address and social security number, when you update the form or give one to another payer. The amounts in Steps 2 to 4 and your choices can differ, so enter them again and check every line in Review. Sign and date it by hand, then give it to your payer.
