Form I-90

I-90 Application to Replace Permanent Resident Card (I-90 form)

By RepeatForm team

Form I-90 is the form USCIS says to use to replace your Green Card. If you have completed an I-90 before and need the same details on a new copy, upload that one and a blank one. USCIS says to check the edition date. You review, sign it yourself, and preview free.

Tested with the uscis.gov edition dated 01/20/25

What Form I-90 is

uscis.gov says: use this form to replace your Green Card. USA.gov and USCIS list reasons to get a new card, such as an expiring card, loss, theft or damage, and a legal name change or other corrected information. Which reason applies to you is chosen on the form from USCIS's list.

Who should not use Form I-90

USCIS says do not submit this form if you are a conditional resident seeking to remove conditions on your Green Card. A conditional resident submits Form I-751 (Petition to Remove Conditions on Residence, for a Green Card obtained through marriage) or Form I-829 (Petition by Investor to Remove Conditions on Permanent Resident Status). If you are not sure which applies, read the USCIS page; RepeatForm cannot tell you.

Download the current form and check the edition

The USCIS page says every form you send must be the current acceptable edition, shown under Edition Date on the form's page (01/20/25 at the time of writing) and at the bottom of each page of the form and instructions. All pages must be from the same edition, or USCIS may reject the form. RepeatForm does not supply the blank form: get it from uscis.gov.

File online or by mail

USCIS says you have two options: online or by mail (paper). To file online you create a USCIS online account, which also lets you pay the fee online, check your case status and respond to requests for evidence. If you file online there is no PDF to fill in. This page is for the paper form. Mailing addresses are on the USCIS I-90 page; USCIS says that if you do not mail it to the right location it may take longer to process.

Going through the form

Part 1 asks about you: your full name (the card is issued in this name), whether your name has legally changed, A-Number if any, USCIS online account number if any, and your mailing and physical addresses. Later parts cover the reason for applying, your signature, and any interpreter or preparer. USCIS says it will reject the form if Family Name, Mailing Address or Date of Birth in Part 1 is missing. RepeatForm does not choose your reason or tell you what evidence to send: see the I-90 instructions. Fees are on the USCIS Fee Schedule (Form G-1055) page.

Using the same details on a new copy

Upload the I-90 you completed and a blank one; RepeatForm carries the matching answers across; you review them, then preview the PDF free.

What carries over

  • ✓Your name, A-Number, date of birth and other personal details that match in both copies
  • ✓Mailing and physical address. Check they are still current, including any apartment, suite or floor number. If you move, you must update your address with USCIS
  • ✓Your status in Part 2 (permanent resident, commuter or conditional resident) can carry. Check it yourself

What never carries over

  • ✓Your signature. The signature boxes stay blank for you to sign. The signing date can carry from your completed copy, so check it and replace it. USCIS says it rejects any unsigned form.
  • ✓Anything USCIS prints and locks on the form, including the barcode. If the barcode would be out of date for the new details, you see a notice: "The barcode on this form won't update. USCIS may ask for a newly printed copy."
  • ✓The reason for applying does not carry: choose it yourself on the new copy.
  • ✓Anything in your completed copy that does not appear in the blank form.
  • ✓Evidence and the fee. Those are separate from the form and are yours to prepare.

Current uscis.gov edition

The Form I-90 PDF on uscis.gov is marked Edition 01/20/25, the same as the Edition Date on the uscis.gov form page. Get the current form from uscis.gov

If USCIS has a new edition

USCIS updates the form from time to time. Carrying an older copy into the current edition works for many answers, but some fields are renamed or moved and match only partly, so check every field after a new edition.

Good to know

  • ✓You review every carried-over answer. RepeatForm does not guarantee the form is correct and cannot promise USCIS will accept it.
  • ✓RepeatForm is not a law firm and does not give immigration or legal advice. It cannot tell you which form or reason applies, whether you are eligible, or what fee you owe. Use the USCIS page and the form instructions.
  • ✓RepeatForm does not send your application to USCIS.
  • ✓The form includes a barcode that RepeatForm cannot update.
  • ✓It works with digital, text-selectable PDFs only. Scans, photos and handwriting are not supported.
  • ✓Same or recent editions work best. If the form changed you may get fewer matches, so check the preview carefully and get the current form from uscis.gov.
