Form 8655

Form 8655, Reporting Agent Authorization

By RepeatForm team

Form 8655 authorizes a reporting agent to sign and file certain returns, make deposits and payments, and receive duplicate notices for a taxpayer. The form names one taxpayer, so each client needs its own. RepeatForm helps reporting agents reuse their own details: upload a Form 8655 you completed and a blank, then review.

Tested with the irs.gov edition dated Rev. January 2024

Is this page for you?

Plainly: this is one of the better fits for payroll providers and reporting agents. You complete a Form 8655 for every client, and the Reporting Agent block is the same each time. The client's details are not, so the earlier client's name, EIN, address and contact will carry too if they match: replace every one. If you complete a Form 8655 only once, the benefit is small.

What Form 8655 is for

irs.gov says "Use this form to authorize a reporting agent to:" "Sign and file certain returns;" "Make deposits and payments for certain returns;" and "Receive duplicate copies of tax information, notices, and other written and/or electronic communication regarding any authority granted;" and to provide the IRS with information to aid in penalty relief determinations. RepeatForm does not choose the authority for a client.

What the form asks for

The form starts with the Taxpayer block (lines 1a to 8) and then the Reporting Agent block (lines 9 to 14). Line 15 asks you to "Indicate the tax return(s) to be signed and filed." Line 16 covers the returns for which the reporting agent may make deposits or payments. Line 17 requests duplicate notices, and line 18 is the disclosure authorization for the W-2, 1099, 3921 and 3922 series. Line 19 is for state or local returns. For the start periods the form gives the format, such as "YYYY/MM" for quarterly returns and "YYYY" for annual returns.

Who signs

The taxpayer signs, and the form says "I certify I have the authority to execute this form and authorize disclosure of otherwise confidential information on behalf of the taxpayer." Its Who Must Sign section lists, for example, "Sole proprietorship. The individual owning the business." and "Trust or estate. The fiduciary." Read that section for corporations, partnerships and LLCs. RepeatForm cannot tell you who may sign.

What the authorization does not do

The form says "An authorization does not relieve the taxpayer of the responsibility (or from liability for failing) to ensure that all tax returns are filed timely and that all federal tax deposits (FTDs) and federal tax payments (FTPs) are made timely." It also says "Any authority granted on Form 8655 does not revoke and has no effect on any authority granted on Form 2848 or 8821, or any third-party designee checkbox authority."

Adding, changing or ending an authorization

The form says "Once Form 8655 is signed, any authority granted is effective beginning with the period indicated on lines 15, 16, 18a, 18b, and/or 18c and continues indefinitely unless terminated or revoked by the taxpayer or reporting agent." To add authority it says "submit another signed Form 8655, completing lines 1–14 and any line on which you want to add authority." A new reporting agent ends the old one: "the filing of a new Form 8655 indicating a new reporting agent terminates the authority of the prior reporting agent beginning with the period indicated on the new Form 8655." The form's Terminating or Revoking an Authorization section has the steps to revoke.

Where to send it

The form has a Where To File section with the address. It says "You can fax Form 8655 to the IRS" and "When faxing Forms 8655, please send no more than 25 forms in a single transmission." Fax numbers and addresses can change, so use the current form and instructions at irs.gov.

Using the same agent details for another client

Upload a Form 8655 you completed and the current blank; RepeatForm carries the matching answers across; you review them, then preview the PDF before you export. Use the earlier form as the source for each client's form, then replace the client's details and change what differs.

What carries over

  • ✓The reporting agent's name, EIN, address, contact person, phone and fax (lines 9 to 14), where they match in both copies
  • ✓Anything else that matches in both copies. This includes the taxpayer's details (lines 1 to 8) from the earlier client, so replace them with the new client's before you export

What never carries over

  • ✓The taxpayer's details (lines 1 to 8). They belong to each client, so enter the new client's yourself and replace any that carried over.
  • ✓The tax returns and the start periods on lines 15 and 16, and the line 18 dates. They differ for each client and RepeatForm does not decide them.
  • ✓The taxpayer's signature, title and date. The client signs and dates by hand where the form asks.
  • ✓Anything the IRS prints and locks on the form, and anything in your completed copy that does not appear in the blank form.
  • ✓Sending the form to the IRS. A completed form does nothing until the signed form reaches the IRS.

If the IRS has a new edition

The IRS publishes a new Form 8655 when the form changes. Carrying an earlier copy into the new one works for many answers, but some fields are renamed or moved and match only partly, so check every line on the new form in Review before you export.

Line 15 lists the quarterly employment tax return, Form 941.

It also lists the annual federal unemployment return, Form 940.

Line 15 also lists Form 943.

The annual employment tax return for smaller employers is Form 944.

A separate authorization for a representative or for tax information has its own page: Form 8821.

For a representative who acts for a taxpayer, see Form 2848.

Good to know

  • ✓You review every carried-over answer. RepeatForm does not guarantee the form is correct and cannot promise the IRS will accept it.
  • ✓RepeatForm does not give tax or legal advice. It cannot tell you which returns, deposits or periods to authorize or who must sign for a client, and it does not decide any start period. The IRS instructions explain those rules.
  • ✓RepeatForm does not send the form to the IRS, get the client's signature or file anything for you.
  • ✓It works with digital, text-selectable PDFs only. Scans, photos and handwriting are not supported.
  • ✓Same or recent editions work best. If the form changed you may get fewer matches, so check the preview carefully and get the current form from irs.gov.

FAQ

FAQ

What is Form 8655 used for?+

irs.gov says "Use this form to authorize a reporting agent to:" "Sign and file certain returns;" "Make deposits and payments for certain returns;" and receive duplicate copies of notices. RepeatForm helps a reporting agent reuse its own details across clients; it does not decide what a client authorizes.

Who has to sign Form 8655?+

The taxpayer signs. The form's Who Must Sign section says, for example, "Sole proprietorship. The individual owning the business." and "Trust or estate. The fiduciary." It has separate rules for corporations, partnerships and LLCs, so read that section for your client.

How do I add to or end a reporting agent authorization?+

To add authority the form says to "submit another signed Form 8655, completing lines 1–14 and any line on which you want to add authority." Filing a new Form 8655 with a new reporting agent "terminates the authority of the prior reporting agent beginning with the period indicated on the new Form 8655." The form's Terminating or Revoking an Authorization section explains how to revoke.

Does Form 8655 replace Form 2848 or Form 8821?+

No. The form says "Any authority granted on Form 8655 does not revoke and has no effect on any authority granted on Form 2848 or 8821, or any third-party designee checkbox authority."
