What Form SS-5 is for

The form itself says to use it to apply for an original Social Security card, apply for a replacement card, or change or correct information on your Social Security number record. It says applying is free. RepeatForm cannot tell you which of the three applies to you.

Do you need the paper form?

ssa.gov says that for a replacement card you may be able to apply online depending on your situation, and if not you need an appointment at a local office. After SSA completes the request, the replacement card arrives by mail in 5 to 10 business days according to ssa.gov. SSA also says you may not need to replace your card if you know your number, because in most cases a physical card is not necessary. The form itself says that if you are age 12 or older and have never received a Social Security number, you must apply in person. This page is for the paper form.

The form on ssa.gov is marked "Form SS-5 (12-2024) UF", and the marker says to discontinue prior editions. RepeatForm does not supply the blank form: get it from ssa.gov.

Name change and other record changes

The form says that to change information on your record, such as a name or citizenship change or a corrected date of birth, you must provide documents that prove your identity, support the change and establish the reason. A document supporting a name change must be recent and identify you by both your old and new names. SSA says it can accept only original documents or documents certified by the custodian of the original record, and that notarized copies and photocopies not certified by the custodian are not acceptable. Documents you mail are returned to you. The form's instruction pages list the acceptable evidence.

Going through the form

The form asks for your name as it will appear on the card, other names used, date of birth, your parents' information, the reason you are applying and a mailing address where you can receive the card. Its instructions say to show an address where you can receive your card 7 to 14 days from now, and that if you apply for an original card for a child under 18 you must show the parents' Social Security numbers unless a parent was never assigned one. Providing race and ethnicity is voluntary. The form says that if you are 18 or older and able to read and complete it, you must sign it yourself; the form lists who may sign for a child or for an adult who cannot sign. RepeatForm does not decide which documents you need: read the form's evidence pages or call SSA.

Using the same details on a new copy

Upload the SS-5 you completed and a blank one; RepeatForm carries the matching answers across; you review them, then preview the PDF free.

What carries over

  • ✓Your name, date of birth and other personal details that match in both copies
  • ✓Parent information and your mailing address. Check that they are still correct
  • ✓The reason for applying can carry too, but it may be different this time. Check it yourself

What never carries over

  • ✓Your signature. The signature box stays blank for you to sign. Today's date (item 14) can carry from your completed copy, so check it and replace it.
  • ✓Anything SSA prints and locks on the form. Those boxes are left exactly as SSA printed them, and you see a notice in the review when this happens.
  • ✓Anything in your completed copy that does not appear in the blank form.
  • ✓Evidence. Original or certified documents are separate from the form and are yours to prepare.

If SSA has a new edition

SSA updates the form from time to time. Carrying an older copy into the current edition works for many answers, but some fields are renamed or moved and match only partly, so check every field after a new edition.

Good to know

  • ✓You review every carried-over answer. RepeatForm does not guarantee the form is correct and cannot promise SSA will accept it.
  • ✓RepeatForm does not give legal advice and cannot tell you which application type or documents apply to you. Use the form's own instructions or SSA.
  • ✓RepeatForm does not send your application to SSA. The form says to take or mail the signed application and your documents to a Social Security office.
  • ✓It works with digital, text-selectable PDFs only. Scans, photos and handwriting are not supported.
  • ✓Same or recent editions work best. If the form changed you may get fewer matches, so check the preview carefully and get the current form from ssa.gov.
