Form 3911

Form 3911, Taxpayer Statement Regarding Refund

By RepeatForm team

Form 3911 is the taxpayer statement the IRS uses to trace a refund check you did not receive, or that was lost, stolen or destroyed. The form says to complete a separate one for each refund. RepeatForm helps if you need a second trace: upload a Form 3911 you completed and a blank, then review.

Is this page for you?

Plainly: this is a lower-fit page. A refund trace is usually a one-off, and the form says "You must complete a separate Form 3911 for each refund for which you are requesting information." This page helps most if you need a second trace, or you are helping someone else with theirs, and want your details from a form you already completed.

What Form 3911 is for

irs.gov says "Form 3911 is completed by the taxpayer to provide the Service with information needed to trace the nonreceipt or loss of the already issued refund check." The form itself says "If you did not receive your refund or if the refund check you received was lost, stolen or destroyed, complete the entire form." RepeatForm does not trace a refund or decide whether you need to file one.

Before you send it

The About Form 3911 page says "If the taxpayer was expecting the refund as a paper check and is filing as Single, Married filing Separate or Head of Household, they are encouraged to begin a trace through one of our self-help methods," and lists Where's My Refund?, the IRS app and tax records and transcripts. Check that page first.

Mailing or faxing it

The About page says "Find the state you live in and use the mailing address, or fax number list below. Do not send anything other than a Form 3911 to the fax numbers below." The addresses and fax numbers are not repeated here because the IRS updates them; use the list on irs.gov. The form's own note says "Return to us in the envelope provided or fax the form."

Who signs

The form says "Sign below. If this refund was from a joint return, both spouses must sign, before we can begin a trace." Line 10 is the signature and line 11 is the spouse's signature, each with a date. RepeatForm carries the dates, but the line 10 signature box does not carry. Anything typed in the line 11 spouse box can carry, so check or clear it in Review. Both spouses sign by hand.

Using the same details on a second trace

Upload a Form 3911 you completed and the current blank; RepeatForm carries the matching answers across; you review them, then preview the PDF before you export. Use one prior copy as the source for each new trace, then change what differs.

What carries over

  • ✓Your name, taxpayer identification number and address (Section I), where they match in both copies
  • ✓Your spouse's name and taxpayer identification number, where they match in both copies
  • ✓Anything else that matches in both copies, such as your phone number. Check it yourself; the refund, the tax period and what happened to the check are different each time

What never carries over

  • ✓The refund itself: the type and amount of the refund, the tax period, the date filed and what happened to the check (not received, lost, stolen, destroyed, or signed). Enter those again for this refund.
  • ✓Bank details. They carry only where both copies match; check them yourself.
  • ✓Anything the IRS prints and locks on the form, and anything in your completed copy that does not appear in the blank form.
  • ✓A signature. The line 10 signature box does not carry. Anything typed in the line 11 spouse box can carry, so check or clear it in Review. Sign by hand; both spouses must sign if the form says so.
  • ✓Sending the form. A completed form does nothing until the IRS has it.

If the IRS has a new edition

The IRS publishes a new Form 3911 when the form changes. Carrying an earlier copy into the new one works for many answers, but some fields are renamed or moved and match only partly, so check every line on the new form in Review before you export.

The return the refund came from may be Form 1040: Form 1040.

Good to know

  • ✓You review every carried-over answer. RepeatForm does not guarantee the form is correct and cannot promise the IRS will accept it or trace a refund.
  • ✓RepeatForm does not give tax or legal advice. It cannot tell you whether to file a Form 3911, which refund to trace, or where to send it. Use the IRS About page and a qualified adviser.
  • ✓RepeatForm does not send the form to the IRS or file anything for you.
  • ✓It works with digital, text-selectable PDFs only. Scans, photos and handwriting are not supported.
  • ✓Same or recent editions work best. If the form changed you may get fewer matches, so check the preview carefully and get the current form from irs.gov.

FAQ

FAQ

How do I file Form 3911?+

The form says to complete the entire form if you did not receive your refund or the check was lost, stolen or destroyed, and to return it "in the envelope provided or fax the form." The About page lists the mailing address and fax number by state. It also says "You must complete a separate Form 3911 for each refund," per the form. RepeatForm does not send it for you.

How can I track my lost IRS refund check?+

The About Form 3911 page lists self-help methods for a paper-check refund: "Where's My Refund?", the IRS app, tax records and transcripts, and a "Lost refund" page. It says taxpayers filing as Single, Married filing Separate or Head of Household "are encouraged to begin a trace through one of our self-help methods." The IRS About page is the place to start.

How can I send a fax to the IRS for a refund trace?+

The About Form 3911 page lists a fax number for each IRS refund inquiry unit by state and says "Do not send anything other than a Form 3911 to the fax numbers below." Use the list on irs.gov for the current numbers.

Can I reuse a Form 3911 I filled out before?+

You can reuse your name, taxpayer identification number, address and phone number where they match. The refund and its tax period are different each time, so enter them again and check every line in Review. The dates carry and the line 10 signature box does not. Anything typed in the line 11 spouse box can carry, so check or clear it in Review. Sign by hand, and both spouses sign if the form says so.
