What Form 4506-T is

irs.gov says to use Form 4506-T to request any of the transcripts: tax return, tax account, wage and income, record of account and verification of non-filing. The transcript format partly masks personally identifiable information while the financial data stays visible, to allow tax preparation, representation or income verification.

Transcripts you can get online first

irs.gov recommends getting a transcript online as the fastest method: sign in to your Individual Online Account. If you cannot register, it describes getting a return or account transcript mailed to you, or requesting one by calling the automated phone service at 800-908-9946. Form 4506-T is for the cases those do not fit, such as a preparer or lender requesting for a client.

What the form can request

The 2025 form lists the return transcript, account transcript, record of account, verification of non-filing, and Form W-2, 1099, 1098 and 5498 series transcripts, with one tax form number per request and the end date of the year or period requested. The form says most requests are processed within 10 business days, and that it must be received by the IRS within 120 days of the signature date.

Reusing details across requests

A lender or processor often types the same taxpayer details and customer file number approach into each form, and a preparer retypes their own details. Upload a 4506-T you completed and a blank one. RepeatForm carries across the answers that match in both copies, you review them, then you preview the PDF before you export.

What carries over

  • ✓The customer file number and the transcript checkboxes, where they match in both copies
  • ✓The taxpayer's names, numbers, addresses and phone can carry too, but they belong to the previous request. Replace every one
  • ✓Anything else that matches in both copies. Check the transcript type, form number and year or period yourself

What never carries over

  • ✓The taxpayer. Names, taxpayer identification numbers, addresses and phone belong to the previous request. Replace every one.
  • ✓The transcript type, tax form number and year or period. These differ for every request.
  • ✓The customer file number. It is optional, up to 10 digits and cannot be a taxpayer identification number; assign it per request.
  • ✓A signature or date. The PDF has no signature or date field. Sign and date by hand, and remember the 120-day rule counts from the signature date.
  • ✓A decision. RepeatForm does not tell you which transcript you need.

Signature and date

The Form 4506-T PDF has no signature field and no date field RepeatForm can fill. They stay blank for you to complete by hand. A taxpayer phone number is a text field and can carry, but it belongs to the previous taxpayer, so replace it.

If the IRS form has a new edition

The IRS renames fields between editions, so carry-over is partial. Check every field after a new edition.

Good to know

  • ✓You review every carried-over answer. RepeatForm does not guarantee the form is correct.
  • ✓RepeatForm does not give tax or legal advice, does not submit the form to the IRS, and cannot tell you which transcript to request.
  • ✓It works with digital, text-selectable PDFs only. Scans, photos and handwriting are not supported.
  • ✓Same or recent editions work best. If the form changed you may get fewer matches, so check the preview carefully.
